Master Accident List for Case Studies

There were three criteria used for the selection of the accidents on the master accident list. Those three criteria were: (a) the accident involved an airplane operated by a scheduled commercial airline (except for the Space Shuttle Challenger accident); (b) the accident resulted from a significant human judgment and decision making error or errors; and (c) there was enough information available in the public domain to develop a comprehensive case study on the accident.
